    This dissertation studies the economic forces that determine prices in business-to-business health care markets. Health care is a large fraction of the US economy. In US health care markets, consumers are often insured and interact with the health care system through intermediary decision makers (health care providers, insurers, and employers). Prices for the associated goods and services in these markets are typically negotiated in business-to-business transactions, which may have complicated implications for consumers. In this dissertation, I analyze pricing agreements across three vertical channels. First, I document large variation across insurers in the prices paid for the same medical services at the same hospital. This price variation, although difficult for consumers to observe, is driven at least in part by insurers' negotiating effort, and materially impacts the value of the insurance plan. Second, in a retrospective analysis of hospital mergers, I document relatively small cost savings, or ``efficiency'' gains post-merger. Where they do exist, cost reductions tend to be concentrated among high-tech implantable devices, and among hospitals for which market concentration in the output market is of the greatest concern. Finally, I study competition in the employer sponsored health insurance market. I estimate a model of supply and demand, which allows me to simulate the introduction of a potential publicly offered insurance plan, or public option, to the employer-sponsored market. I find that a public option has limited competitive impact on competition in the employer sponsored market under most policy scenarios, due to existing price discrimination and wide variation in the cost of providing insurance to different employer groups.

    The first chapter, co-authored with Allyson B. Root and Benjamin R. Handel, studies how information and behavioral nudges impact patient behavior in end-of-life care. Despite the substantial economic and personal implications of end-of-life health care decisions, many fail to document their wishes or select a representative to make medical decisions on their behalf. Descriptive evidence suggests that this can result in sub-optimal outcomes including dissatisfaction and unnecessary medical spending, but it is not well understood why patients fail to engage in this high-value planning. We conduct an initial and subsequent intervention to facilitate advance directive (AD) completion in the patient population of our partner, Providence St. Joseph Health (PSJH). Using a randomized control trial, we find a significant 5 percentage point increase in AD completion with physical letter reminders tied to future primary care appointments, doubling the completion rate in the patient population from the start of the study. In addition, we find that including the physical AD form with paper letters as a nudge to decrease hassle costs increases AD completion 9 percentage points compared to no intervention. Our evidence also suggests that these interventions are more effective for older individuals, who are also less sensitive to the type of intervention. Back-of-the-envelope calculations suggest that it would cost $38 for every additional AD form completion using paper letters and included AD forms, compared to costless electronic reminders. However, we find no significant effects in AD completion from the initial intervention involving in-person AD drives and electronic videos.In the second chapter, co-authored with Benjamin R. Handel, Lynn M. Hua, and Yuki Ito, we study health plan choice and health plan menu design with 13 years (2008-2020) of health claims and health plan choice data from the California Public Employees’ Retirement System (CalPERS). We develop a choice model that predicts the number and type of subscribers moving across plans under different plan environments, as a function of (i) plan premiums, (ii) plan cost-sharing, and (iii) plan brand. We find that (i) subscribers overweight premiums relative to out-of-pocket spending by a factor of roughly five to one, in line with other literature, and (ii) subscribers place meaningfully differential values on plan brands. We find that these preferences, especially for plan brand, depend crucially on whether a subscriber's family is sick or healthy. We also find some evidence of risk adjustment blunting adverse selection in our market while it was implemented from 2014-2018, although due to inertia the effects were relatively mild. Finally, we present counterfactual scenarios for future enrollment with and without risk adjustment and inertia.

    This dissertation consists of two chapters of in-depth empirical analyses in the domain of public economics, and one chapter of causal inference methodology regarding distributional robustness. The first chapter is titled ``The Effect of Air Pollution on Academic Performances: Evidence from South Korea, " and investigates the causal effect of air pollution on academic performances. While the effect of air pollution on health has been extensively studied, little is known about its effect on education, especially in a causal context. Here, I exploit the unique geography of Korea and a meteorological phenomenon called Asian Dust Storm (ADS) to get exogenous shocks of air pollution carried by the wind from China. Using two stage least squares regression, I find that an increase in particulate matter (PM10) leads to an increase in the share of students who underperform, while its effect on the share of students who overperform is not different from zero. I find similar results for elementary and middle school test outcomes, and find that air pollution disproportionately affects the types of schools associated with low socioeconomic status. Looking at both short term and long term effect of air pollutants, I find that air pollution has both acute and cumulative effect on the academic performances of the students. I explore health as a mechanism through which air pollution affects academic outcomes, and find that the most detrimental effect comes from the most harmful to health pollutants—PM10 and ozone—and do not find any evidence of preemptive absenteeism or mobilization due to air pollution on the school level. The second chapter, co-authored with Mark Duggan, Irena Dushi, and Gina Li, is titled ``The Effects of Changes in Social Security's Delayed Retirement Credit: Evidence from Administrative Data." The delayed retirement credit (DRC) increases monthly OASI (Old Age and Survivors Insurance) benefits for primary beneficiaries who claim after their full retirement age (FRA). For many years, the DRC was set at 3.0 percent per year (0.25 percent monthly). The 1983 amendments to Social Security more than doubled this actuarial adjustment to 8.0 percent per year. These changes were phased in gradually, so that those born in 1924 or earlier retained a 3.0 percent DRC while those born in 1943 or later had an 8.0 percent DRC. In this paper, we use administrative data from the Social Security Administration (SSA) to estimate the effect of this policy change on individual claiming behavior. We focus on the first half of the DRC increase (from 3.0 to 5.5 percent) given changes in other SSA policies that coincided with the later increases. Our findings demonstrate that the increase in the DRC led to a significant increase in delayed claiming of social security benefits and strongly suggest that the effects were larger for those with higher lifetime incomes, who would have a greater financial incentive to delay given their longer life expectancies. The third chapter, co-authored with Hongseok Namkoong, is titled `Robust Causal Inference Under Covariate Shift via Worst-Case Subpopulation Treatment Effects." In this chapter, we propose the worst-case treatment effect (WTE) across all subpopulations of a given size, a conservative notion of topline treatment effect. Compared to the average treatment effect (ATE), whose validity relies on the covariate distribution of collected data, WTE is robust to unanticipated covariate shifts, and positive findings guarantee uniformly valid treatment effects over subpopulations. We develop a semiparametrically efficient estimator for the WTE, leveraging machine learning-based estimates of the heterogeneous treatment effect and propensity score. By virtue of satisfying a key (Neyman) orthogonality property, our estimator enjoys central limit behavior---oracle rates with true nuisance parameters---even when estimates of nuisance parameters converge at slower rates. For both randomized trials and observational studies, we establish a semiparametric efficiency bound, proving that our estimator achieves the optimal asymptotic variance. On real datasets where robustness to covariate shift is of core concern, we illustrate the non-robustness of ATE under even mild distributional shift, and demonstrate that the WTE guards against brittle findings that are invalidated by unanticipated covariate shifts.

    The gap in equity for women in leadership roles continues to be disparate from the representation of women in the workforce and even greater for women in healthcare. Equity in leadership in healthcare is driven by policies, procedures, and programs to intentionally promote and hire women into leadership roles. This study explored drivers of leadership equity through the Clark and Estes (2008) gap analytic framework in looking at assumed knowledge, motivation, and organizational needs towards decreasing organizational gender barriers for women to advance into leadership roles. This study was conducted with women in healthcare in various roles that support a community hospital facility. A review of literature explored gender barriers through history, and across themes that impact women’s advancement into leadership roles. The influences were identified as either assets, partial gaps, or gaps through semi-structured interviews. Findings revealed needs in areas of knowledge, motivation and a critical support theme emerged from the organization as a barrier to equity. Based on these findings a set of recommendations was provided to support both women and organizations to support aspiring women leaders. This study serves as a first step for organizations to understand the experiences of women aspiring to healthcare leadership and provides structured actions to improve these experiences as well as impact the gender gap in healthcare.

    Understood as a benefit derived from social security, healthcare provision was historically developed in Mexico within the context of labor laws and tied to the multiple social security institutions (SSIs) created throughout the first half of the twentieth century. However, the combination of an increase in informal unemployment, lack of institutional fiscal frameworks to support revenue raising, and political-economic aspects led to the development of a three-tiered system with financing and healthcare delivery practically independent from each other: 1) public health services provided by the various social-security institutions to the SSI-affiliated population, 2) public Ministry of Health (MoH) delivery for the population without affiliation to SSIs (nonSSI population) and 3) an increasingly growing private sector that complements the public health system’s inability to meet population needs.During the past three decades, Mexico adopted multiple health reforms targeting the nonSSI tier with the aim of reducing inequities that result from financial and healthcare delivery fragmentation across the two tiers of the public healthcare system. These reforms included changes to health financing, workforce and shifted the fiscal attributions between states and the central government. While there exists abundant evidence on the consequences of these reforms on health system performance of the public non-SSI tier, two issues remain unexplored. First, the variation of these effects across the 32 Mexican states. Second, the absence of studies in the private sector even when almost half of national total health spending is financed through private funds.In this context, this dissertation aims to assess the participation of the private sector in health financing and workforce for the non-SSI tier of the Mexican healthcare system. To do so, it carries out three empirical studies with different research designs and data sources.The first study uses a cross-sectional design and data from an individual-level survey of adults from four Latin American countries in 2014; Mexico, El Salvador, Colombia and Brazil to investigate the association between private insurance and financial protection. Results point out that countries that managed to integrate the social security and non-social security health tiers through changes in health financing arrangements, –Brazil and Colombia– are potentially better able to provide financial protection to their population than Mexico and El Salvador, which continue to face steeper integration challenges underpinned by their health financing arrangements.The second and third studies are sub-national analyses of the effect of public health spending for the non-SSI population on financial protection (study 2) and workforce (study 3). Study 2 uses a cross-sectional design (2018) to test if states that allocate a higher level of financial resources to health for the non-SSI population are better able to protect their population against financial burden. Overall, results point to an inverse relationship between public health spending and financial protection and that this is even more pronounced in states that allocate a higher share from their own revenues.Study 3 uses multiple available data sources to build 17-year panel (2004-2020) to investigate the effect of public health spending for the non-SSI on changes in the number of physicians working in public and private settings. Results show that the rate of nurses grew more than physicians during the period of study, particularly among those working in public settings. While public health spending on the non-SSI population contributed to increased public and private workforce, there are steep variations across states.This dissertation contributes to the discussion of central government-state relations as well as the interaction between public-private sectors in healthcare, since they can no longer be independently understood.

    With ongoing changes in mortality and fertility, numerous social challenges have accompanied population aging. This dissertation draws on data from the United States and the United Kingdom to examine the social dimensions of family and care in the context of population aging. In the first chapter, I use data from the UK Household Longitudinal Study to explore differences in intergenerational co-residency between UK-born and foreign-born adults. I find that immigrants, regardless of age at migration, are less likely to co-reside with parents than UK-born individuals, suggesting structural barriers to migration and kin availability shape multigenerational household formation, with implications for family care. In the second chapter, I use data from the Survey of Income and Program Participation (SIPP) to examine the personal care needs and care arrangements of individuals 65 and older living in the United States, focusing on how life stage at migration impacts aging-related care. I find that immigrants who migrate to the United States after age 50 show distinct care needs and care arrangements, likely driven by barriers to health and retirement supports. In chapter three, I use data from the SIPP to explore the relationship between unpaid caregiving and women’s labor market participation in the context of changing demographics of care. Given patterns of population longevity and later ages of childbearing, more individuals are exposed to overlapping child and adult care responsibilities across the life course. I examine connections between “sandwich caregiving” and labor market participation among women living in the United States and find that sandwich caregivers who are younger, caring for young children, and providing “high intensity” adult care face the largest labor market penalties. I discuss these findings against the backdrop of eroding social supports for both child and adult care. My dissertation presents new evidence aimed at exposing the fragility of existing safety-nets for long-term care, and I argue that, without a critical examination of social policies and practices that organize long-term care, population aging will continue to exacerbate inequalities by gender, race, and immigrant status.

    This dissertation explores the impacts of shocks to healthcare costs and access on households and firms.The first chapter studies the impact of recent changes in abortion access on household finances. I exploit variation in changes in access to abortion resulting from Planned Parenthood clinic openings and closings during the period 2010-2019 to study the relationship between access to abortion and financial health (delinquencies). I find that auto loan delinquencies are negatively related to access to abortion, a relationship which is unique when compared to other types of loans (i.e., credit cards, mortgages, and student loans). I show that this effect is driven by changes in access in low-income areas, shedding light on the mechanism as well as the significance of auto loans, which are disproportionately held by low-income individuals (e.g., compared to mortgages). I provide further evidence of the causality of the relationship between access to abortion and auto loan delinquencies by exploiting a 2013 legislative change in Texas. Finally, I use these estimates to calculate the impact of the overturning of Roe v. Wade in June 2022 on financial health.The second chapter explores the connection between rising healthcare costs and declining small business employment and startup rates. I use private equity (PE) acquisitions of hospitals during the period 2002-2017 as a shock to local healthcare prices in order to study the effect of increasing healthcare costs on the composition of firms in local economies using a difference-in-differences framework. I use confidential Census Bureau data to show first that PE acquisitions of hospitals indeed affect the health insurance premiums charged by insurers to local employers. Turning to the economic implications, I show that smaller firms face both increased barriers to entry and decreased firm resilience following a PE-induced increase in health insurance premiums; firms with 20 or fewer employees are less likely to enter the market, while firms with between 11 and 50 employees who do still enter face a higher probability of exit.This dissertation has important policy implications, showing the significance of recent trends in healthcare to both firms and individuals.
