    Das Corpus Hippocraticum enthielt in der Antike eine (heute verlorene) chirurgische Schrift mit dem Titel "Über lebensgefährliche Verletzungen und Geschoßextraktion", welche Erotian, der Hippokratesglossator der Nerozeit, ebenso wie Galen noch kannten. In der vorliegenden Arbeit werden sämtliche sekundär überlieferte Fragmente und Testimonien (hierunter ein bislang noch nicht beschriebenes) übersichtlich zusammengestellt und im Kontext der inhaltsverwandten antiken Überlieferung diskutiert. Textstellen bei Celsus, Galen und Paulus Aegineta, die das verlorene Werk mutmaßlich rezipieren, werden von philologischer wie von medizinischer Seite intertextuell analysiert. Diese Textpassagen erlauben in Zusammenschau mit den Testimonien und Fragmenten eine annähernde inhaltliche Rekonstruktion der verlorenen Corpusschrift, in der unter anderem Anweisungen zur Anlage einer Bauchnaht gegeben wurden. Insgesamt ist festzustellen, daß sich hippokratische Chirurgie, anders als bislang angenommen, nicht allein auf das Gebiet der Orthopädie beschränkte, sondern gleichfalls die Therapie schwerwiegender Weichteil-, Bauch- und Organverletzungen, mithin das ganze damalige Spektrum nicht-elektiver Chirurgie (Notfallchirurgie) umfaßte

    Abstract: Einige klinische Studien lassen vermuten, dass Regionalanästhesie das Risiko chronischer Schmerzen nach chirurgischen Eingriffen verringern könnte.

    Ziel der Arbeit
    Unser Ziel war es, Regional- und Lokalanästhesie mit konventioneller Schmerztherapie in Bezug auf die Verminderung des Risikos der Entwicklung chronische Schmerzen bei Erwachsenen und Kindern drei Monate nach chirurgischen Eingriffen zu untersuchen.

    Suchmethoden
    Wir führten unsere Datenbankrecherche auf PubMed, in Embase und in der Cochrane Library CENTRAL durch. Wir schlossen nur randomisierte klinische Studien einmit mindestens eine Laufzeit von mehr als drei Monate nach elektiven chirurgischen Eingriffen bei Kindern oder Erwachsenen untersuchen.

    Extraktion und Analyse der Studiendaten
    Mindestens zwei Personen extrahierten die Studiendaten und evaluierten die methodische Qualität unabhängig voneinander. Wir legten uns a priori auf ein „Random-effects“-Modell fest.

    Hauptergebnisse
    Wir fanden 63 RCTs, konnten aber nur 39 Studiendaten verwenden. Unterschiede im Studiendesign und unzureichende Daten erschwerten unsere Evidenzsynthese. Das Verzerrungspotenzials aufgrund fehlender Daten, Publikationsbias und mangelhafter Verblindung mindert den Evidenzgrad unserer Metanalyse.

    Schlussfolgerungen
    Regional- und Lokalanästhesie das Risiko anhaltender Schmerzen 3 bis 18 Monate nach offen thorakalen Eingriffen und 3 bis zwölf Monate nach Kaiserschnitt reduziert (mittlerer Evidenzgrad) und vermindert das Risiko chronischer Schmerzen 3 bis zwölf Monate nach Brustoperationen (niedrige Evidenzgrad). Die kontinuierliche intravenöse Infusion von Lokalanästhetika kann das Risiko für chronische Schmerzen nach Brustoperationen ebenso vermindern (mittlerer Evidenzgrad)
