    There is significant interest in understanding the labor market consequences of the opioid epidemic, but little is known about how opioid use impacts on-the-job productivity. We analyze the impact of opioid initiation in the emergency department (ED) on workforce outcomes in the Military using linked medical and administrative personnel data for active duty service members from 2008 to 2017. Exploiting quasi-random assignment of patients to physicians in the ED, we find that assignment to a high-intensity opioid prescribing physician increases the probability of long-term opioid use and leads to subsequent negative effects on work capacity, job performance, and productivity. We also analyze the mechanisms underlying these negative workforce outcomes. While opioid use does not negatively affect measures of physical job performance, we find large increases in behavioral problems which lead to disciplinary actions and job separation

    This paper introduces four new intergenerational and multigenerational datasets which follow both sons and daughters and which can be used to study the persistence of longevity, socioeconomic status, family structure, and geographic mobility across generations. The data follow the children of Black and White Union Army veterans from birth to death, linking them to the available censuses. The White samples include an over-sample of children of ex-POWs. A separate collection links grandchildren of White Union Army veterans to their death records. The data were created with high quality manual linkage procedures utilizing a wide variety of records to establish links

    One explanation for insufficient use of primary care in the U.S. is a lack of trust between patients and providers - particularly along racial lines. We assess the role of racial concordance between patients and medical providers in driving use of preventive care and the implications for patient outcomes. We use unique data from the Military Health System, where we observe providers as patients so that we can identify their race, and where moves across bases change exposure to provider race. We consider patients with four chronic, deadly, but ultimately manageable illnesses, where the relationship with the provider may have the most direct and important impact on health. We find striking evidence that racial concordance leads to improved maintenance of preventive care - and ultimately lower patient mortality. Pooling across these diseases, we estimate that a one-standard deviation increase in the share of providers who are Black leads to a 15% relative decline in Black mortality among those with these manageable illnesses. Our results further suggest that between 55 and 69% of this mortality impact arises through improved medication use and adherence, with other aspects of the provider-patient relationship accounting for the residual

    Much of the increase in the prevalence of overweight and obesity has been in developing countries with a history of famines and malnutrition. Prior research has pointed to the association between overweight and famine exposure during developmental ages as one of several explanations and has hypothesized that ancestral famine exposure may play a role. Data limitations have prevented researchers from investigating the multigenerational effects of famine shocks on descendant overweight. This paper examines overweight among adult grandsons of grandfathers exposed to starvation during developmental ages. I study grandsons born to grandfathers who served in the Union Army during the US Civil War (1861-5) where some grandfathers experienced severe net malnutrition because they were POWs during times of extreme hardship. I find that male-line but not female-line grandsons of grandfathers who experienced a severe captivity during their growing years faced a 21% increase in mean overweight and a 2% increase in mean BMI compared to grandsons of non-POWs. This increase was mediated by fathers' and to a lesser extent own in-utero conditions, as proxied by season of birth, suggesting a dynamic process of inheritance. Male-line grandsons descended from grandfathers who experienced a harsh captivity faced a 22-28% greater risk of dying every year after age 45 relative to grandsons descended from non-POWs, with overweight accounting for 9-14% of the excess risk

    As millions of soldiers deployed to Iraq or Afghanistan between 2001 and 2021, Veteran Affairs Disability Compensation payments quadrupled and the veteran suicide rate rose rapidly. We estimate causal effects of combat deployments on soldiers' well-being. To eliminate non-random selection into deployment, we leverage quasi-random assignment of newly recruited soldiers to units on staggered deployment cycles. Deployments increase injuries, combat deaths, and disability compensation, but we find limited evidence that they affect suicide, deaths of despair, financial health, incarceration, or education. More dangerous deployments have similarly limited effects. Our estimates suggest that deployment cannot explain either the recent rise in disability payments, which is more likely driven by policy changes, or the surge in noncombat deaths, which is better explained by shifts in observable characteristics of soldiers

    Lack of information and advising prior to college matriculation may contribute to poor post-secondary outcomes among non-traditional students. We conducted a large-scale, multi-arm field experiment with the U.S. Army to investigate whether a package of research-based personalized information and access to advising affects postsecondary choices and attainment among a large non-traditional adult population. We find no impact of the intervention on whether veterans enroll in college, on the quality of their college enrollment, or on their persistence in college. Our results suggest that influencing non-traditional populations' educational decisions and outcomes will require substantially more intensive programs

    This research is the first to identify the impact of armed conflict exposure for the average male randomly drawn from the population on subsequent intimate partner violence (IPV). We exploit a population-level natural experiment in service location assignment of draftees under Turkey's universal conscription system, inducting 90% of all draft-age men for 15-to-18 months, with nearly a quarter of them being deployed to the conflict zone during our analysis period, 1984-to-2011, in the southeast of the country to curb the Kurdistan Workers' Party (PKK) insurgency. Purging any confounding influence of civilian exposure, the innovative design of our survey captures isolated exposure during military service. Results show that conflict zone deployment increases physical and psychological IPV perpetration from husband to wife. Probing the mechanisms, our analysis first renders the use of violence as an instrumental behavior in intrahousehold bargaining as an unlikely mechanism by eliminating labor market outcomes and economic- and social-controlling behaviors from the list of usual suspects. Moreover, we rule out the possibility of risky health habits exacerbating the unfavorable effects of combat. Then, we show compelling evidence that normalizing violence in everyday life, likely emerging as an expressive behavior when arguments escalate, is the primary mediating pathway

    This paper investigates the empirical relationship between inclusion and state capacity, as theorized by Besley and Persson (2009). We examine the impact of racial discrimination on Black U.S. military enlistment during the onset of WWII. We find that discrimination had a large and negative effect on volunteer enlistment after the Pearl Harbor attack. The result is robust to a large number of controls that account for potential confounders. The negative effect of discrimination is moderated by geographical proximity to Pearl Harbor, and is larger for educated men. We provide consistent evidence for Japanese Americans

    We show that armed actors refrain from using their power to arbitrarily steal from an economy if, and only if, the armed actors' property rights over stealing from that economy are secure. By 2009, armed actors taxed, administered, and protected various villages in Democratic Republic of the Congo. We exploit the timing and targeting of an international military operation that permanently made taxing these villages impossible. Following the operation, these armed actors turned to violently expropriating the same villages. The findings suggest that the security of property rights over stealing, hence the stealing horizon, can sustain, or destroy, economic growth

    How do elites mobilize commoners to participate in a war? How does war mobilization affect elite power after the war? We argue that these two questions are interconnected, as elites mobilize war often because war benefits them. We demonstrate these relationships using the setting of the organization of the Hunan Army - an army organized by one Hunanese scholargeneral that suppressed the deadliest civil war in history, the Taiping Rebellion (1850-1864). We construct comprehensive datasets to depict the elites in the scholar-general's pre-war network as well as the distribution of political power before and after the war. By examining how pre-war elite connections affected where soldiers who were killed came from, and subsequent shifts in the post-war distribution of political power toward the home counties of these very elites, we highlight a two-way nexus of elites and war mobilization: (i) elites used their personal network for mobilization; and (ii) network-induced mobilization elevated regional elites to the national political stage, where they influenced the fortunes of the country after the war

    We use variation in exposure to victimization of 1,537 households of eastern Congo for each year of 1990-2013 to examine the formation of preferences to participate in armed groups. In this context, most armed groups are Congolese militia, whose objective is fighting foreign armed groups. We find that foreign armed group attacks on household members are associated with a larger propensity that individuals join a Congolese militia in subsequent years. The results are consistent with the formation of preferences arising from parochial altruism towards the family to fight foreign perpetrators. Specifically, we find that the effect is driven by the most gruesome of those attacks, by those that take place at a young age, and persists for several years. Consistent with parochial altruism, we find that the effect is largest when the victim is a household member or the village chief, smaller when the victim is another household in the village, and insignificant if the victim is in a nearby village. To examine the external validity of our result, we analyze heterogeneous effects by weakness of the state. We find that the response is concentrated in village-year observations in which state forces are absent. Finally, we show that, to undo this effect, the yearly per capita income outside armed groups would have to permanently increase 18.2-fold. These results suggest that intrinsic preferences are important for armed group participation relative to economic incentives, and emphasize their interaction with state weakness

    "This book examines the figure of the returning warrior as depicted in the myths of several ancient and medieval Indo-European cultures. In these cultures, the returning warrior was often portrayed as a figure rendered dysfunctionally destructive or isolationist by the horrors of combat. This mythic portrayal of the returned warrior is consistent with modern studies of similar behavior among soldiers returning from war. Roger Woodard, Ŵs research identifies a common origin of these myths in the ancestral proto-Indo-European culture, in which rites were enacted to enable warriors to reintegrate themselves as functional members of society.     We investigate the relationship between opioid diverting policy and suicides among the veteran population. The opioid epidemic of the past two decades has had devastating health consequences among U.S. veterans and military personnel. In 2013, the Veterans Health Administration (VA) implemented the Opioid Safety Initiative (OSI) with the goal of discouraging prescription opioid dependence among VA patients. Between 2012 and 2017, prescription opioids dispensed by the VA fell 41% (VA, 2018). Because this involved the aggressive curtailing of opioid prescriptions for many VA patients, OSI may have had a detrimental effect on veterans' mental health leading to suicide in extreme cases. In addition, because rural veterans have much higher rates of VA enrollment, more prescription opioid use and abuse, and lower rates of substance abuse and mental health treatment utilization, we expect any effect of OSI on veteran suicides to be concentrated in rural areas. We find that OSI raised the veteran suicide rate relative to the non-veteran ("civilian") rate with rural veterans suffering the lion's share of the increase. We estimate that OSI raised the rural veteran suicide rate by a little over one-third between 2013 and 2018

    We study public vs. private provision of health care for veterans aged 65 and older who may receive care provided by the US Department of Veterans Affairs (VA) and in private hospitals financed by Medicare. Utilizing the ambulance design of Doyle et al. (2015), we find that the VA reduces 28-day mortality by 46% (4.5 percentage points) and that these survival gains are persistent. The VA also reduces 28-day spending by 21% and delivers strikingly different reported services relative to private hospitals. We find suggestive evidence of complementarities between continuity of care, health IT, and integrated care
